3. Patient Portal Accounts

4. Clinic Services

Service information displayed on the platform is for information and access guidance only. Availability, fees, consultation requirements, eligibility, medical suitability, and appointment arrangements may change and remain subject to clinic confirmation.

5. Donation Support Items

Donation support items are clinic or foundation-supported entries published by authorised administrators. They may include fixed amounts, no fixed amount, verification fees, optional fees, mandatory fees, or reward values configured by the clinic.

Submitting a donation request does not create automatic approval, entitlement, medical priority, or guaranteed reward. All requests remain pending until reviewed by an authorised administrator.

6. Verification and Rewards

• Users may be required to submit amount paid, payment reference, proof file, and supporting notes.
• The clinic may approve, reject, request clarification, or investigate any submission.
• Rewards are credited only after successful administrative approval.
• Fraudulent, duplicate, misleading, or unverifiable submissions may lead to rejection, account restriction, or further action.

7. Payments, Fees, and Refunds

Users must use only official clinic payment details displayed in the portal or otherwise issued directly by the clinic. Users are responsible for checking payment details before making payment.

Refunds, reversals, corrections, or adjustments are subject to clinic policy, applicable law, verification status, administrative review, and the nature of the relevant payment or donation.

8. Complaints and Concerns

Patients and users may raise concerns or complaints through the clinic’s official contact channels. Healthcare providers in England are expected to maintain effective complaints procedures for care-related concerns.

9. Prohibited Use

• Uploading false, forged, harmful, unlawful, or misleading documents.
• Attempting to bypass verification, authentication, or administrative controls.
• Using the platform for fraud, spam, abuse, scraping, unauthorised access, or disruption.
• Misrepresenting identity, payment status, medical status, eligibility, or authority.

11. Limitation of Liability

To the maximum extent permitted by applicable law, Civora Care Clinic, its staff, partners, administrators, and representatives shall not be liable for indirect, incidental, consequential, or punitive damages arising from platform use, delayed verification, user error, third-party service failure, or unauthorised access caused by user negligence.

The clinic may update these terms when necessary. Continued use of the platform after changes means you accept the revised terms.
